Home Care Packages
Our goal is to provide quality occupational therapy services to our clients, so that they can continue to be safe, confident and independent in performing their daily activities at home.
Falls risks increases as we age, therefore it is important to take care of ourselves and prevent falls in our home environment.
In addition to completing functional and home safety assessments, the occupational therapist can also address the areas of the client’s concerns and provide advice, prescribe aids, equipment and home modifications to reduce the risks of falls at home.
We provide assessments to support the clients’ independence and safety whilst living at home. For example, the occupational therapist will enquire about your daily routine and areas that you may have difficulties coping at the moment.
The occupational therapist will also assess areas inside and outside your home that can may have potential falls hazards. We also work closely with family members and carers to identify the needs of the clients. Recommendations will be made according to the client’s individual needs.
After completing the assessments, the occupational therapist will recommend assistive technology such as recliner lift chairs, shower stools, bedside commode and wheelchairs to enable you to complete your daily tasks safely with increased confidence and independence.
Driving a powered mobility device (PMD) eg. a mobility scooter can increase an older adult's independence and confidence and enable them to participate in their daily tasks at home and in the community. A mobility scooter is beneficial for the older adults who have weakness in their lower limbs or who have reduced tolerance in their mobility.
The client will be assessed on their cognition, physical/psychosocial skills and driving lessons (trial of mobility scooters) will be provided to ensure that they are safe. The client will also have a driving assessment to assess their competence and safety before the occupational therapist can determine if they are safe to use the PMD.
Having a grab rail in the bathroom or a handrail at the front access can provide support, reduce the risks of falls and improve the safety of the client. Some examples of home modifications include threshold ramps, step ramps, platform steps and non-slip tapes in the shower recess to keep you safe at home, improve your confidence and independence.
